What this page covers.

This policy is about www.bosschiropractictx.com, the website. Your medical record at the office is separate. It is protected health information under federal law, it is governed by the practice’s Notice of Privacy Practices, and you get a copy of that notice when you come in. Ask the front desk for it any time.

What the appointment form collects.

The booking form asks for your name, phone number, email address, a preferred day and time, whether you are a new patient, and a short description of what brings you in. Only the first three are required.

Please keep that description short and general. “Low back pain for three weeks” is enough for us to schedule you. Do not send test results, medication lists, insurance numbers, or anything you would not say out loud in a waiting room. The detail belongs in the exam room, where it is protected properly.

The form goes to the office so someone can call you back and book a time. That is the only thing it is used for. We do not sell it, rent it, or trade it, and we do not add you to a marketing list because you filled it in.

How the form actually gets to us.

The site is hosted by Netlify, and the form is handled by Netlify Forms. That means the message sits on Netlify’s servers on its way to the office inbox. Netlify is a normal web host, not a medical records system. That is the reason for the short-and-general rule above.

What the website measures.

The site uses Google Analytics to count visits and see which pages people read. That tells us whether a page is doing its job. It records things like the page you looked at, roughly where in the world you are, and what kind of device you used. It does not know your name.

If you would rather not be counted, most browsers have a setting for this, and Google offers an opt-out add-on. Turning it off does not change anything about how the site works for you.

Things the site loads from elsewhere.

• Google Maps, on the contact page, so you can get directions.
• Google Fonts, for the typefaces.
• A 3D spine model, which loads its graphics library from a public code host.

Each of those companies sees that your browser asked them for something. Their own privacy policies cover what they do with that. We do not send them your name, your phone number, or anything you typed into the form.

Cookies.

The site itself does not set advertising cookies and does not follow you around the internet. Google Analytics sets its own cookie to tell a repeat visit from a new one. Your browser settings control all of it.

Children.

This website is written for adults. We do not knowingly collect information through it from anyone under thirteen. If a child has filled in the form, call the office and we will delete it.

Asking us to delete something.

Call (281) 681-2677 or write to the address below and ask. If it is a form submission, we will remove it. If it is part of your medical record, different rules apply and the office will explain what we can and cannot do, because records law tells us how long some things must be kept.

Security, honestly stated.

The site is served over HTTPS, which encrypts what you send. No website can promise perfect security, and anyone who tells you otherwise is selling something. Email and web forms are not the right place for sensitive detail. The phone is.

If this changes.

We will change the date at the top of this page. Anything significant will be said plainly rather than buried.
