What this page covers.

This policy is about www.bosschiropractictx.com, the website. Your medical record at the office is separate. It is protected health information under federal law, it is governed by the practice’s Notice of Privacy Practices, and you get a copy of that notice when you come in. Ask the front desk for it any time.

What the appointment form collects.

The booking form asks for your name, phone number, email address, a preferred day and time, whether you are a new patient, and a short description of what brings you in. Only the first three are required.

Please keep that description short and general. “Low back pain for three weeks” is enough for us to schedule you. Do not send test results, medication lists, insurance numbers, or anything you would not say out loud in a waiting room. The detail belongs in the exam room, where it is protected properly.

The form goes to the office so someone can call you back and book a time. That is the only thing it is used for. We do not sell it, rent it, or trade it, and we do not add you to a marketing list because you filled it in.

How the form actually gets to us.

The site is hosted by Netlify, and the form is handled by Netlify Forms. That means the message sits on Netlify’s servers on its way to the office inbox. Netlify is a normal web host, not a medical records system. That is the reason for the short-and-general rule above.

What the website measures.

We use Google Analytics 4 to see how visitors use the public pages of this site. It shows us which pages people read, which buttons they tap, how far down a page they scroll, and roughly where in the world visitors are. It does not know your name.

We may also use Microsoft Clarity on the public pages. Clarity shows us anonymous replays of how people move around a page. It does not know your name either.

If you tap OK on the cookie notice, we also use the Meta pixel on the public pages. It tells Meta (Facebook and Instagram) which page you visited and whether you tapped the phone number or the Book button, so we can see which ads work and show our ads to people who have already visited. It never sees what you type. If you do not tap OK, it never loads.

None of these tools is used on the booking page or the thank-you page. Nothing you type into the appointment form is sent to Google, Microsoft or Meta.

When you arrive, the site also notes which page you landed on and which site sent you. That note stays in your browser. If you send the booking form, it goes to the office with the form, so we know how you found us. It is not sent anywhere else.

You can say no. Tap No thanks on the cookie notice at the bottom of a page and the tools stop. You can change your choice any time with the button below. Your browser settings and Google’s opt-out add-on work too. Turning measuring off does not change anything about how the site works for you.

Things the site loads from elsewhere.

• Google Maps, on the contact page, so you can get directions.
• Google Fonts, for the typefaces.
• A 3D spine model, which loads its graphics library from a public code host.

Each of those companies sees that your browser asked them for something. Their own privacy policies cover what they do with that. We do not send them your name, your phone number, or anything you typed into the form.

Cookies.

Google Analytics sets cookies named _ga to tell a repeat visit from a new one. If we use Clarity, it sets its own cookies for the same reason. Both are for counting. If you tap OK, the Meta pixel sets cookies named _fbp and _fbc, which are advertising cookies: they let Meta show our ads to people who visited. If you tap No thanks, we stop all of them and clear the Google and Meta cookies we can reach. Your browser settings control all cookies.

Children.

This website is written for adults. We do not knowingly collect information through it from anyone under thirteen. If a child has filled in the form, call the office and we will delete it.

Asking us to delete something.

Call (281) 681-2677 or write to the address below and ask. If it is a form submission, we will remove it. If it is part of your medical record, different rules apply and the office will explain what we can and cannot do, because records law tells us how long some things must be kept.

Security, honestly stated.

The site is served over HTTPS, which encrypts what you send. No website can promise perfect security, and anyone who tells you otherwise is selling something. Email and web forms are not the right place for sensitive detail. The phone is.

If this changes.

We will change the date at the top of this page. Anything significant will be said plainly rather than buried.
