Three different sleep problems.

Trouble falling asleep. Waking through the night and not getting back down. And waking up because something hurts. They look similar from the outside, and they are not the same problem. Falling-asleep trouble often points toward stress, anxiety, an irregular schedule, or habits like screen use and caffeine late in the day. Trouble staying asleep has a wider range of causes, including sleep apnea, restless legs, and pain. Waking specifically because of pain is the one a chiropractic office has any business looking at, and it is worth being precise about which of the three you actually have.

What commonly causes it.

Beyond stress and habits, sleep apnea is one of the most common medical causes of broken sleep, and it involves the airway partially or fully collapsing during sleep. Restless legs syndrome causes an uncomfortable urge to move the legs that is worse at rest. Thyroid problems, certain medications, caffeine or alcohol close to bedtime, and long stretches of anxiety or low mood all disrupt sleep as well. Pain that flares when you shift position at night is its own category, and it is usually mechanical, coming from a joint, disc, or nerve in the spine.

What the signs typically look like.

These signs point toward a medical sleep problem:

  • Difficulty falling asleep most nights
  • Waking multiple times without an obvious reason
  • Snoring loud enough for a partner to notice
  • Gasping or pauses in breathing during sleep
  • An urge to move your legs at night
  • Daytime sleepiness that affects driving or concentration

Waking at the same point every time you roll onto a particular side, or a leg going numb in certain positions, points more toward a mechanical cause in the spine.

What a doctor typically checks.

A physician will ask about your sleep schedule, how you feel during the day, and whether a partner has noticed snoring or breathing pauses. Screening for anxiety, depression, and thyroid problems is common. When sleep apnea is suspected, a sleep study is the standard way to confirm it. Medication review is often part of the visit too, since several common drugs can disturb sleep as a side effect.

When the spine is part of it.

Some sleep trouble follows a mechanical pattern: you fall asleep fine and then wake every time you roll over, your neck aches by four in the morning and never settles, or a leg goes numb in certain positions. Those are mechanical patterns, and an exam can tell us what is behind them and whether it is something we treat. When to see your doctor

See your doctor if you snore heavily, gasp or stop breathing in your sleep, or if a partner has noticed it, since that can point to sleep apnea. Go too for any of these:

  • Daytime sleepiness bad enough to affect driving
  • Sleep problems that have lasted more than a month
  • A new sleep problem after starting a medication
  • Sleep broken by chest pain or shortness of breath
  • Low mood that comes with it

Numbness, weakness, or loss of bladder or bowel control needs urgent medical care.
